The first thing worth trying is restarting Niagara. In other reports of duplicated entries that cleared them, and they tend to turn up right after an app updated itself; some apps change the identity Android reports for them when they update, which is a known limitation on our side.
If both entries are still there after a restart, could you check whether they open the same app? Two entries that open different dialers would be two separate apps sharing a name rather than a glitch. Your Niagara version would help as well, it is shown at the bottom of the all-apps list under Launcher Settings > Advanced: Checking Niagara Launcher App Version.
